Summary (from Wikipedia)

First we flash forward to find out Jack is sharing ahousehold with Kate and Aaron.

Meanwhile, as Sawyer, Claire, Aaron & Miles head for the beach and the Losties, Miles hears whispering in a small forest clearing and asks about Rousseau and Karl. Then, Miles as Miles brushes a little of the dirt in the clearing away he uncovers their very shallow graves.

At the same time, at the beach, the Losties are losing their patience with Daniel and Charlotte. WHen Jack tries to cool tempers he passes out: Jack has a hot appendix and it needs to come out! Juliette sends Sun, Jin, Charlotte, and Daniel to the medical station for supplies: they can’t risk moving Jack, his appendix might rupture.

Flashforward: Dr. Stillman from Santa Rosa calls on Jack. He’s trying to treat Hurley but Hurley doesn’t think Stillman exists. When Jack visits Hurley, Hurley Hurley says that all the Oceanic six are dead — ‘we never got off the island’. Jack tells Hurley he changed his mind about seeing Aaron after Kate’s trial. Hurley admits he was happy for a while [after the O6 got back but then he started seeing Charlie. In fact, Charlie told Hurley Jack would be coming to visit and told Hurley to Jack a message, “You’re not supposed to raise him Jack.”

Back at the island, Sawyer, Claire, Aaron & Miles are still treking through the jungle to re-group with the main Losties group at the beach when they encounter Frank Lapidus. Frank tells them to hide — Keamy are his mercernaries are right behind him. At least one of the mercernaries is injured. Frank helps Sawyer’s group escape detection by the mercernaries claiming they need to get back on the helicopter before dark.

In the future, Jack returns to the home he shares with Kate after his visit with Hurley. Jack proposes and Kate accepts.

Zoom: Back to the island for a SURPRISE! Charlotte speaks fluent Korean. Jin tells Charlotte - in Korean - to get his wife off the island.

With the needed supplies, Juliette begins surgury with Bernard’s help. Kate is on hand at Jack’s request to hold a mirror so he can see what’s happening. But the surgery is much more painful than Jack realizes and Bernard knocks him out with chlorophorm. Kate is booted out of the temporary operating theator. After the surgery Bernard comes out after the surgery to let Kate know that everything went well. When Kate peeks in to see Jack, Juliet tells Kate that she thinks thinks Jack still loves her.

FLASH FORWARD: Jack is back in the real world, a practicising physician. Late one night, Jack is dictating notes when he sees his father. The moment is interrupted when a colleague walks by — Jack looks back and his father is gone. Jack gets the other doctor to write him a prescription for sleeping pills

THE ISLAND: on the island. Claire wakes in the middle of the night. She’s been sleeping by campfire but Aaron is missing. She looks across the fire to see that Christian Shepard is holding her baby.

FLASH FORWARD: Kate arives home in the evening to find Jack has gotten home early and has been drinking. He wants to know where Kate has been. “Trust me, leave it be,” she asks, but Jack presses. Kate confesses she was doing something for Sawyer, “I made him a promise”. She won’t tell Jack what it was because, Sawyer wouldn’t want her to. Jack becomes even more angry: Saywer made his choice, he stayed, “I’m the one who saved you.” When Kate complains about Jack’s behavior in front of Aaron Jack shouts, “Your son! You’re not even related to him”.

The epiosode ends back on the Island. Sawyer wakes up to find Claire missing from their camp site. Miles said she “Walked off into the jungle in the middle of the night”, and that Claire wasn’t alone, “she called him Dad.” Sawyer shouts for Claire and begins running only to hear the baby crying. He finds Aaron alone, in the jungle with no sign of Claire.
