Summary (from Wikipedia)

This episode features flashbacks from John Locke’s life. Among other things, Locke was born prematurely to a teen mother and was contacted more than once in his childhood and teen years by Richard Alpert and/or the Mittelos Labs.

Trechery abounds on the freighter: Capt. Gault helps Zayid take the ship’s zodiac to the island. Keamy shows himself to be ruthless. One of Keamy’s mercernaries tells the ship’s doctor that the morse code message from the island revealed the doctor’s body had come ashore on the island. When Frank refuses to fly the mercernaries back to the island Keamy kills Ray, the doctor, (by slitting his throat) and throws the doc’s body off the ship.) Frank quick changes his mind.

Back at the island, John, Hurley and Ben search for Jacob’s cabin. John, Hurley and Ben find the cabin but Ben refuses to go inside — Ben says, his time “is over”. Hurley refuses to go inside as well. So John enters the cabin alone and sees a figure in shadow seated inside. No, I’m not Jacob, “but I can speak on his behalf”. “Who are you? I’m Christian [Shepard]. Do you know why I”m here? Yeah, sure — do you? I’m here because I was chosen to be. That’s absolutely right.”

Lo and behold: Claire is in the cabin as well — seemingly in a happy mood. “I’m with him,” she says, meaning Shepard. (But something isn’t right with Claire: is she alive? Dead? In between?)

John asks: “How do I save the island?”

Christian on behalf of Jacob tells John that “they need to move the island.”
